If you have expressed interest in Transportation on your Volunteer Information Form, please complete the Transportation Agreement.
Thank you for caring about our Alaskan Klee Kai in need and welcome to our transport system. Volunteers are what make this work.
The AKK you are transporting should NEVER be allowed off leash unless he/she is in your safely fenced yard, securely shut in your home, or securely shut in a crate.
Please feed only the food and the amounts recommended.
Be sure to use a crate when the dog is to be left alone. This will insure the dog’s safety, along with the safety of your home and/or vehicle.
Should the AKK you are transporting become ill, PLEASE NOTIFY ALASKAN KLEE KAI NATIONAL RESCUE IMMEDIATELY. In case of an extreme emergency such as injury or severe illness, take the dog to the nearest available veterinarian immediately, AND NOTIFY ALASKAN KLEE KAI NATIONAL RESCUE IMMEDIATELY.
Do not allow anyone to take the dog from you, except the next scheduled leg transporter at the designated location.
I agree to indemnify and hold harmless Alaskan Klee Kai National Rescue Inc., its officers, directors, employees, volunteers, agents, and representatives for all manner of actions and causes of actions, suits, debts, dues, accounts, bonds, covenants, contracts, agreements, judgments, claims and demands whatsoever arising out of my possession and placement of the dog. Alaskan Klee Kai National Rescue Inc., makes neither warranty nor guaranty, expressed or implied, with regard to the physical condition of the dog, its background, suitability or compatibility. I also agree to surrender the dog to the Alaskan Klee Kai National Rescue Inc., at any time if requested.
I hereby agree to adhere to the above transport rules and provide the safest and best care I can provide for the dog I am entrusted to transport.
